There are some cards usable 2-3 times, place an activation token on them, and discard the card after so many uses or tokens are on them. Is the cost paid to put the card into play for all uses, or does the cost have to be paid each time it is used, until it is full and discarded?

you only have to pay pay once. after that you get to activate X times.
