I had the unusual scenario where every PC and NPC ended up at initiative 2.
What is the rule if everyone in the engagement has the same initiative, who goes first? Are attacks made simultaneously, therefore regardless if a monster is killed he still gets to attack back?

"When hero markers and NPC or creature markers appear on the same initiative order, heroes act first, then NPCs and creatures."
