Do you pay the resource once and discard the Card .

Or do you have to Pay the resources every time you want to refresh your Hero ?

Every time you want to refresh. Bring Miner of the Iron Hills!

You only need to pay each Refresh phase. If you have other means of readying (such as Unexpected Courage), you do not have to pay extra to use those effects.
