I have been on the fence on picking up Descent due to the limited co-op. With the release of the Apps, I am considering picking it up. Besides the base game is there anything up front I need to get? (besides the apps) I had heard to use the apps you needed to buy the co-op packs as well.

No, you just need the base game and the app.
Although I have heard that there is a bug with the app currently where some quests/monsters from expansions you don't own may show up. I would expect this to be fixed 'soon' though.

I solved this problem by buying all the expansions...lol.

As far as co-op dungeon crawler board games go, this is by far the best. Definitely pick up at least the base game and give it a try with the app, It is tons of fun. And playable alone if you're into that thing, it's nice to be able to try out all the heroes and skills and figure out what good hero combos are.
The app is free, the co-op campaigns are free so far. So you just need the base game and you're good to go.
The additional stuff you can pickup if you want to later, and when you do you just make a note on the app, then the app will randomly pull bits from your additional sets into the adventure.

Every descent expansion can be used in RTL except for the co-ops (Nature's Ire,..).
