Just curious - has anyone ever rolled 3 Triumphs or 3 Despairs on a single check? If so, how did you adjudicate it?

Yes, 3 Triumphs and I automatically wiped out the bad guy and his henchmen. It was the end of the evening and the GM said it was a good way to end the session.
It's pretty much whatever makes you feel good, or real bad, imo.

a player of mine rolled 3 triumphs when slicing the systems of some ties that were chasing their starship. he basically took over the controls of some and had them crash into each other, wiping out a whole bunch of them.
another time our big badass wookie had to roll a fear check when he was confronted by vader (though it was just hologram, but he didn't know that). he rolled so bad that he simply fainted, though im not sure if it was really 3 or just 2 despairs.
As GM, I had a bad guy who was one of the best bounty hunters in the galaxy, and his thing was to use a Heavy Verpine Shatter Rifle at Extreme range to soften his opponents up, sometimes shooting their weapons.
He had a particular hatred for Jedi or other people carrying lightsabers.
Of course, he had also stolen the identity of another bounty hunter who had previously been the mentor of one of the PCs.
And anytime that PC was looking for a particular piece of gear but he didn’t roll any net successes, the explanation would be that the last one of that type of gear had just been bought by — guess who?
Needless to say, I wasn’t rolling for obligation here.
Anyway, in one particular combat, where I had set up multiple large minion groups to go up against the PCs, enough that they would be a serious challenge and even stood a good chance of winning against the PCs, this same fake-mentor bounty hunter was watching from Extreme range, and periodically plinking them. But he only targeted their weapons. He had helped set up the jailbreak/riot, and he wanted to soften the party up some.
Of course, the lead Jedi had recently finished constructing a greatly improved lightsaber. And I rolled three Triumphs on the attack of the fake-mentor bounty hunter.
Given what this guy had been doing (and the fact that he’d already shot the lightsaber out of this same guys hand at least two or three times previously), it didn’t make sense for that roll to do anything else but to actually destroy a lightsaber. Two triumphs is supposed to be enough to destroy any weapon, but then a lightsaber is supposedly indestructible.
I didn’t want to take out the newly constructed lightsaber, so I ruled that a different lightsaber that the PC was also carrying was the one to be severely damaged — not technically destroyed, and repairable, but no longer currently useable.
Then I discovered that the PC was carrying that other lightsaber in his backpack, and not on his waist.
I left the choice up to the player, and while he felt that it didn’t really make sense for the other lightsaber to be the one that was damaged, I think he wanted to keep his main lightsaber functional more than he wanted a ruling that made more sense.
So, yeah — I’ve done that. On three Triumphs (or three Despairs rolled by the PCs), if it makes sense for the motivations of the NPCs and the situation, then I will destroy or at least heavily damage a prized possession like a lightsaber.
3 triumphs could do just about anything, lol.
Mechanics check? Permanent bonus maybe, maybe they have enough parts to build a second thing, etc.
Athletics check? Recover strain, wounds, and find something important or valuable.
Charm check? They become so infatuated with your character that they end up doing whatever your character wants for the rest of the session. An NPC helper could be useful.
3 dispairs should be really, really bad. It's hard enough to roll one and you could only roll that many of an opposed check.
Melee check? A player could automatically lose a limb, and have their weapon destroyed.
Negotiate check? You didn't read the fine print on the agreement you signed when you sold some parts. Now you own them money, services, etc and the group gets added obligation (if no obligation than your group will have a nagging menace to deal with).
Discipline check? The character faints, takes wounds, and now suffers from PTSD that requires a fear check whenever triggered. It can go away eventually but would linger for a while.

How did he manage that? Did you make the player take an aim and get the two black dice? Also, you could let the force user roll a force die and if the result is favorable manage to maintain their grip.
I remeber the "Healer" forcesensitiv rolling 3 Triumphs on while trying to heal an critcal wound of a teammember... I ruled that in a burst of warm light all wounds (normal an critical) of all teammembers got healed at onced (regardless of distance, pips, strength or magnitude upgrades)
it was an call of the Lightside for the heroes to be on the right path... well later on the player that should be cured intantionally got hit hardly by a vicios weapon an lost his right arm... karma is a bi**h sometimes ^^.
I've seen it once and then I saw 4 triumphs a few weeks later! (The odds on that one are pretty crazy). I made a house rule that if you get 3 triumphs then you gain +1 skill rank in a somewhat related skill (GM's choice), but you have to describe it. In my case, the pc wanted to do a stealth check to sneak up on and take out a guard. He described his character approaching from behind, doing a ninja flip over the guy's head and stabbing him in the head as he went over. I granted him a +1 in acrobatics to encourage him to try fun things like that in the future. Now he is our group ninja and the party vividly remembers that moment.

The BH in question had max attributes and max skill, and lots of bonuses due to equipment and talents. His Ranged Heavy and Gunnery were literally as good as they could get in this game. So, he was rolling really, really well. I didn’t have enough dice to do the entire roll in one fell swoop, so I had to split the number of dice in half and do two parts. Each time he fired, he managed to get more Successes and Advantage in the first half of the roll that he didn’t even need the second half of the roll in order to hit. But then that second half of the roll came in like a hypersonic missile.
If he had been firing at the PCs themselves, as powerful as they were, they would still have been toast. But I cranked him up to eleven so that I could have him do things like shooting at their weapons, and just softening them up a bit. He was trying to set them up for live capture at a later time, and he wanted them taken down a few pegs before he got there.
He wasn’t Vader — heck, he wasn’t a Force user at all — but he was as close to Vader as I was willing to get in that campaign, at that time.
I rolled 4 trirumph on the last encounter of our campaign my group wrapped up a month or so ago.
The campaign arc was that our group had joined the rebellion and set out to take down a Grand Moff in a system. For the grand finale of the campaign, we decided to blow up the Moff's flagship as a symbolic gesture to rally the people to the rebellion moreso than just taking out the Moff (since we knew if we killed her the Empire would just replace her).
Anyway, there was an Inquisitor-type dark side force user that had been hounding us for a bit. We had succeeded in planting explosives onboard the ship, and as we were making our way back to our escape shuttle, the Inquisitor stepped out to confront the face-guy of our group. I had rolled very successfully on a stealth check on the way back to the shuttle so the Inquisitor did not perceive me and attacked my group member. On my turn, I stepped out of the shadows, told the Inquisitor, "Bad move", then rolled to unload with 2 upgraded pistols with full Gunslinger tree talents and 4 Ranged-Light skill / 6 agility. Upgraded the roll with Destiny Point. Rolled 4 triumphs and a crapton of advantage. As it was the last combat and pretty much last encounter for the entire campaign, we adjudicated it that my gunslinger basically just blew the guy away. It was pretty epic.

The Uber-BH in question was really more window-dressing than anything.
Beyond just occasionally making things a bit more difficult for the PCs because he was plinking their weapons from Extreme range, he wouldn’t have taken direct action against them for a while.
I just wanted it running around in their heads that there really are guys out there that are that much better than they are, and that will remain the case for the foreseeable future. And as they get more powerful, that target on their back just gets bigger and brighter.
But you’re not the first person on this forum to make that request of me.
You’re welcome to ask my players if they see things the same way, but at this stage I doubt I’ll be doing any more GM’ing, at least for a very long time. I had already taken the campaign about as far as I could before we went on hiatus, and now that my work is … interfering … much more than usual, I’m not in any kind of position to run a game for at least the next several months.
So, I think you’re probably fairly safe. At least, fairly safe from me, that is.
