How does MHD-19 medical loadout skill work?

It says Search the supply deck for a medical card then claim that card.

Do you flip card over one at a time until you hit a medical card?

Or do you pickup the deck and choose the card?

Search generally means - look through it and pick the one you want.

So just pick what you want out of it, then reshuffle the deck.
