I presume that the Corp player can play his ice into a server already rezzed.

If so, do the sub-routine(s) become active immediately (especially if they favour the Corp player)?

E.g. The corp player plays MINELAYER into a sever rezzed. Does he benefit from the sub-routine: "You may install a piece of ice from HQ as the outermost piece of ice protecting this server, ignoring all install costs"?

Why would you assume you can do this?

The rules are very clear about how ICE works. It is installed unrezzed. Subroutines only resolve during an encounter, and only if left unbroken by the Runner.

Other than that, there are some specific exceptions, but the cards that create those exceptions will explain how to handle them.

Remember that the rules for a game explain what you CAN do. If it's not in the rules, you can't do it.

ICE is always installed unrezzed. ICE can only be rezzed during a run. Subroutines only trigger during a run of they are unbroken by the runner.

There are exceptions to this, but the cards will tell you what they are.
