Your campaigns running jokes.

We had a situation where our psyker had burned his torso and was out on the ground dying. I was the chirurgeon of the warband and rolled a medicae. The result was me saying "He is dead" to the rest of the team. Thankfully the Inquisitor was on his way and saved our psyker from my incompetence :P.

Well, first and foremost, I find myselff as the GM find on the receiving end of a kind of running gag of my players: whatever the name of a re-occuring NPC or place is, two of my players will change it slightly and turn it into a joke. I tried to counter that already, so I let them create lists of names for certain things (Inquisitors, voidships, etc.), but, you know, I cannot remake each and every name of things and people in pre-written moduls *sighs*

The other thing: when an imperial NPC is found guilty of some minor wrongdoing, he is likely to be sentenced to "kneel on the market place, in prayer and in public" by our Sister of Battle. This started with a different set of characters (but same gaming group) on the world of Dusk, but continues since.

Oh man, NPC names. I use online name generators for my NPCs (almost nobody has a name until the PCs ask) and 90% of the time it ends up being a d*ck joke. I try to vet them before voicing the name but I think my players are more creative than I am.

I may be playing with manchildren.

Using NPCs as baits for elaborate traps, or "cunning" plans. And then forgetting about them until a few planets later.
