Does this mean 1 Threat symbol is automatically generated..

1 per die?

Or does it mean that no matter what is rolled, A threat result will always happen?

When figuring out the die pool result, ADD one threat.

It is not one per die.

And all the other modifications too. (Damage, defense or soak)

After rolling, but before canceling, you will always have an extra threat added to the results. You can then cancel from there.
