Correction: someone has lost a BODY PART in every movie. Darth maul lost his lower torso. Although I "suppose" you could just call that losing his legs...
I believe ep2 has the most lost body parts of all the movies:

Two tusken heads
A couple geonosians were bisected
Both of the acklay's front legs
3po's head - twice!
Jango's head
Anakin's arm

Kylo Ren in an unfortunate accident. That's what happens when you go with weird saber designs.
Not positive but it seems that somewhere between Return of the Jedi and The Force Awakens C-3P0 lost an arm and had it replaced. The action figures from Force Awakens has his left arm as reddish brown. Really curious if they are going to address it in the movies or books.
3PO also no longer has a silver leg from the knee down. Wonder if the comic will explain that too.
