When TLT is paired with the BTL-A4 title card, does this mean that the Y effectively gets to roll 3 seperate attacks against the same target?

Yes and no

You can make a primary attack followed by the two TLT attacks - but the primary weapon attack does not have to be against the same target as the two attacks from the TLT
