The top of page 2 of the FAQ you were linked to directly answers the question of when it happens. The top of page 11 of the FAQ has additional information about Accuracy Corrector that directly addresses the rest of your question.

(Post 1) He asked a question. That's fine, it's one that has caught a lot of people out over the last few months.
(Post 2) I told him exactly where to go to find the information.
(Post 3) He claimed he'd already seen the FAQ, although he clearly hadn't, because he reiterated exactly the same question.
(Post 4) dbmeboy directed him to the same place in the FAQ.
(Posts 6, 8, and 10) He claimed he didn't know the FAQ had been updated, because he was used to the way Games Workshop does things. Again, it's clear he didn't click through to the link in post #2.
And you're right: there's not quite anything wrong here. Someone who was genuinely interested in the topic probably would have clicked on the FAQ link and scrolled to page 2 to see what I was talking about, but okay. And it doesn't exactly take a degree in library science to know that a document with a date and version number attached is likely to change fairly often, but sure. And this thread (from the dakkadakka archives linked by treybert in another thread) seems to indicate that his problem with GW was that they updated their FAQ too frequently, but that's fine.
But how often should we extend the benefit of the doubt? (This isn't a rhetorical question--it's something I find myself wondering a lot.)

He was just asked to read the FAQ and replied that he thought the FAQ was unreliable. I see distressingly little evidence contrary to the warning.

For what it's worth: the FAQ is always the most up to date resource. No matter how old it is, there is no more up to date or authoritative source. Thus, you should always check it first.

The FAQ is updated roughly twice a year, usually around March-April and September-October. It currently covers all released material to date and generally FFG aim to release new waves of ships around a month or so prior to FAQ updates, so they can see what problems have come up with new ships or cards and then update the FAQ accordingly.
And once again, GW and FFG are world's apart with their game designs and player support. NEVER ASSUME that because GW did something one way, that every other game company will do it that way also. Especially FFG, because in my 35 years of gaming, I've found that FFG seems to be the only guys that do it properly.
