Start of Each Round: The player with initiative places 2 damage tokens next to the Cantina for each Command card in his hand.

End of Each Round: Until the end of the next round, the player who controls the Sabacc Table is considered to have 1 additional VP for each damage token next to the Cantina.

Do the damage tokens accumulate every round?

The damage tokens stay where they are, so they do accumulate.

So, for example, the first round, the player with initiative would place 6 damage tokens next to the Cantina (3 Command cards).

The second round, the player with initiative would add 4 more tokens next to the Cantina (2 Command cards), for a total of 10 damage tokens. (4 + 6)

The third round, 4 more tokens are added for a total of 14 damage tokens. (4 + 4 + 6).
