Once won is this just a constantly applied to wounded heroes from then on, or do I (as the IP) have to 'play' it in some way?

Once Old Wounds has been won, it is immediately put into play as a reward, exactly like rebel rewards and XP upgrades you would buy from your Imperial Upgrade Deck.

No special actions need to be taken.
