I'm guessing that judging by the mission title this is probably triggered by an event that I've missed, but I can't find a mission card for the 'Captured' mission.

Can anyone confirm if there is one?

This is triggered by an end mission event.

We got to the Captured mission in one campaign. It's a result of a particular story mission that, itself, isn't always played every time.
