We have Patrice, and for the first time in 60+ games we have actually run out of Clue tokens. Do we use substitutes, or not?
What happens when you run out of Clue tokens?
Every investigator with Clues is devoured, add doom tokens on the GOO's track equal to the number of investigators thus devoured

I haven't yet hit the bottom of the Clue bag myself, though my first IH game I did catch glimpses of it. Everyone seemed to be getting encounters (AH or OW) that gave them Clues. Another time was the only game where Prof. Rice-Rice-Baby made an appearance.
I never use substitutes. If there are no clues left, my players are out of luck; and seriously, if all 48 clues are on the board or with Investigators they don't need any more. However, for all cards that put clues on them to mark something (rumors, tasks/missions, personal stories) I substitute these with Eihort's Brood Tokens.
A possible different solution: if a clue token is needed but none is left, take it from anywhere else on the board (chosen by the first player). Although that would probably make it to easy to gain the clues from Innsmouth and Dunwich.

This question was only ever answered with regards to a specific League scenario that could give out A LOT of clues. For the purposes of that scenario, when you ran out of clue tokens, the Investigators stopped getting clues until you spent some, but the other things in the game that used clue tokens to count things (such as tasks and rumors) began using other tokens for the duration of the shortage. That was a special rule just for that scenario, but it is also a "worst case scenario" so you might want to go with it.

Only in the Arkham League are clue tokens a finite resource. However, that is not so in the normal game. Especially now with Innsmouth adding more unstable locations and more clue token appearances, yet we have not been given any more than the initial 48.
