Yes otherwise it would say on the card to bring it in exhausted.
I'm guessing yes, it's readied as the card doesn't specify it needs to be exhausted.
Cards always enter play ready unless otherwise specified, right? Can't think of any example contrary.
The answers that cards enter play ready (unless something specifically says otherwise) is in the definition of "Ready."

I suppose 99% of the time this will be used as Combat Action. In other phases it only makes sense for units that 'do something' on deployment/committment. There is no Action Window in Command Phase, and if you use the action in Deployment, you trash the card before Command phase anyway.

There IS one in the command phase, but it's after all Command Struggles have resolved, so you'd need a VERY specific use for it. Perhaps a unit with another Action on the card which you want to fire before the Combat Phase begins? Can't think of any right now, but I could see there being some potential uses for it later on.
