This is one of the eternal questions on this forum!
Some people think that it's inconcievable to play with anything less than the Full Monty.
Some people prefer to mix in one expansion at a time, in order to maximize a particular theme. If you use only the base game and King in Yellow, and play against Hastur and the KiY herald, it makes for a very yellowish game, with the Act cards a serious threat and lots of theatrical stuff going on, which is satisfying. The same is true for the other small box expansions.
Personally I think that if you mix everything in, you dilute some aspects of the game too much, so that quite a few 'dead' cards turns up - cards which form parts of game mechanics which will never pose a serious threat, so you can ignore them and you'll probably get away with it. For example, the Act cards, the Corruption cards, those special state things (like the Coded Messages) and particularly Dunwich gates.
My way of doing it is to mix in everrything you can mix in without diluting things - in some cases this means mixing in very specific parts of expansions. For example, most of the Other World encounters from small box expansions can always be mixed in because all they do is add more variety. The same mostly goes for items, allies, skills etc.
(There is also a more practical reason for how I cut cards from the game: I only include as much stuff as can fit in the base game box, without the plastic tray of course. If you add every card to every deck it's almost impossible to get it all to fit.)
However, when Innsmouth comes out, I at least will try a few games with every single thing mixed in and all three town boards.

It's still playable but you have a couple main issues... Acts aren't a threat and Dunwich gates tend to be somewhat irregular. Normally I play with everything, although I have been considering a minimythos deck variant based on Tibs' thread (I think it was Tibs').
My group usually plays with all the expansions thrown in. Avi_dreader is correct that Dunwich and the King in Yellow tend to fade into the background. That can bother some folks, but overall we enjoy not knowing what's coming.
Ultimately, only you and your group can decide what's most fun for you. Happily, Arkham is a very flexible system and can be played all sorts of ways.
