The definite Squad Mode rules reference

I have made an attempt to consolidate all we now about Squad Mode RAI, as evidenced by the erratas and various official replies to rule questions, in one single article. The whole thing had been a complete mess and I am trying to restore some sanity here for those who didn't follow the discussions back then and don't want to dig through the archives. This is supposed to be the definite reference for DW gamemasters (in absence of an errata v1.2):

While the blog entry features significant parts from the Core Rulebook on squad modes, I hope this isn't a problem for FFG as there is still enough left out to make the quoted stuff unusable without the Core Rulebook. It only serves to benefit GMs with the rulebook. [if it's too much for FFG, I'll edit it or take it down, ofc.]

If some of the veterans here, as service to the community, could take a look and confirm for correctness, completeness and coherence?

I suppose he could, provided he is in Squad Mode. I doubt that you are excluded forever once you leave range. But there is no official ruling as far as I know, so everything is a house rule.

Personally I'd make you pay to rejoin it, in part because the RAW Cohesion rules pretty much let your hoard it to ridiculous levels. I don't feel bad in making players spend that hoard.
