I think anyone that's been in this hobby for a few years has either seen this happen or had it happen to them. That's partly why I don't start going crazy on my character's backstory (especially in games known for their lethality) until after the first couple sessions.

Heck, I've seen a PC die during character creation, thanks to the random life history tables in an earlier version of Traveller.

Naw, with Paranoia you've at least got a good chance of getting out of the briefing room. How many clones you have left after that point is another story. As a friend of mine said, "if your group of troubleshooters actually manages to complete the mission, then you have utterly failed as a Paranoia GM."

Actually, the old sci-fi RPG 'Traveller' had that. Your character could actually die curing creation.

And in Paranoia, as the saying goes, "Its easy to kill troubleshooters. The hard part is getting them to kill each other."

I have a 4e character who died *twice* during character creation. The process of refining the concept mechanically went through a hybrid Gnoll vampire/monk, into a Revenant (still gnoll) Fighter/Barbarian. Two deaths, and still kicking...

Yeah the first time a group I was in tried Traveller only one member of the party survived chargen. After that we swiftly added houserules to prevent death in chargen.

In Aquelarre RPG you can die or suffer severe mutilations while your are creation your character XDD

I remember a game my friends and I used to play back in Highschool. I think it was called "Dead Earth." From what I understand, people claim it to be one of the worst table top RPG systems ever created. But, I disagree. I think it was just ahead of its time, and I think it was perhaps a little sloppy-joe in a few areas. But anyway...

In this game, during character creation and at specific points throughout your character's life-time, you would roll on this list of like 1,000 different Mutations. It was actually pretty impressive. Anyway, these Mutations would do all kinds of stuff to your character. Some would outright kill you, or give you some ridiculous Mutation that you wish it would have just killed you. Some were really great, though...
